    Was scheduled for a crown procedure on May 19,2022. At the time of my appointment, it was determined that they could not seat the crown because my wisdom tooth needed to be removed first, even though they had already taken x-rays prior to this appointment. I left that day without the crown seated and instead a temporary cap was placed. They billed my insurance company for the full amount of service ($1230), to which my insurance covered $199, and I owed the rest. I received numerous calls and letters for payment owed including financing charges, even though the procedure was not completed. I have now received an intent to file a lawsuit from a lawyer if I do not pay in full within 10 days. I feel that they have received payment for services completed to date, and thusly should not be submitting any bills to collections.

    Patient was seen in office on May 19, 2022 to initiate treatment. Patients tooth was prepared for a crown. Patient was advised ideally she should have her wisdom tooth (directly behind the treated tooth) extracted due to possible insertion issues. Patient agreed and stated she would see oral surgeon to have this tooth extracted. Patient left our office with referrals for oral surgeons in the area. Over the last 15 months since patient was in, she has not followed through with this tooth extracted. Our office has tried to contact her multiple times over this time frame, as you will see in the attached progress notes, with no follow through from patient. Our office has indeed done everything we could to follow through with this patient to complete treatment but she has NOT been compliant. We have incurred material fees, lab fees, as well as other in office fees that are included in this charge, which is why we have moved forward with collection attempts. We feel we have exhausted all means of having patient in to complete treatment. If you require any additional information, please feel free to contact me at *************.
