    the social worker we see given did not help us at all she left us stranded and we lost my fathers long term care i called the supervisor and he didn't care at all he had us waiting 3 days for a call, i had to call him myself to be able to put a complaint

    My wife an I went to our yearly wellness exam. The insurance refuses to pay for the bloodwork for any of us. I have been going back and forth between the doctor's ****** and the insurance and still the insurance is not paying for it. The doctor's ****** filed the claim as a wellness checkup but the insurance won't pay. Every time I call the insurance company they keep talking about the explanation of benefits and how this is not covered even though its a wellness exam and should be covered by law

    Issue occurred on Wednesday March 13th, 2024. Sunshine Health was my Medicaid provider on this date. Their ********************** they assigned me to was Modivcare. They drove me to 5353 No. Federal HWY. in Ft Lauderdale for a doctor ************ Their assigned driver who picked me up broke my rollator. I am disabled and need my rollator to ambulate. Without it I cannot walk and have had to give up my independence. I have to call neighbors to take e to the bathroom and help me go to the shower. It is embarrassing. I cannot leave my home. I filed a complaint with Sunshine Health and Modivcare their ********************** They know I am disabled and need my rollator to ambulate. They were asked to replace my rollator their employee broke. They kept telling me supervisors would call me back and no one had. I kept calling and nothing. It has been a month and half now and they will not have a manager contact me to resolve this issue. I need them to replace my rollator they broke. The rollator was $200.00 that I had to pay for it when I originally purchased it. I am asking for them to give me that money so I can purchase another rollator and become human again and be able to move on my own and leave my home. My ID number with them is **********
