      Went in for a Military dental check up today. The Dentist was pushing a not needed deep cleaning. They first refused to complete the military form. However he changed his mind and briefly checked my teeth. I had no cavities, or issues besides cleaning. He said he would score me a 3 which is the lowest grade. A three states that a emergency condition is possible withing 12 months. This also gives the options to choose. None of the options state cleaning as a reason for scoring a three. He also said once I pay for the deep cleaning he would score me a higher grade. I refused to proceed and kindly left the buisness. I have not recieved the bill for this proceedure because they did not give me one. This should not be charged to me or my issurance.

      The patient was seen in our office on 3/20/2024 needing a form DD 2813 ( these are available online for an example)  for the military completed. I have filled out many of these forms over the course of my career as I was a dentist in the service as well before moving to private practice. The patient had dental x-rays taken and an oral exam completed to determine where the next steps of action should be directed. There were no signs of dental caries but obvious signs of periodontal issues. He needed a full mouth debridement (FMD) to rid him of calculus buildup and stain from not having a cleaning for 2 years. This is different than what he stated about me "pushing" a "deep cleaning" he stated that I told him. He did not need scaling and root planning (SRP), only a full mouth debridement (FMD). In addition the form he needed filled out basically places your dental/oral health into 3 categories. The first being class 1 (no dental problems and do not expect any treatment or evaluations for 12 months), class 2 ( there are problems but no one expects any emergencies for 12 months if left alone) and class 3 ( there are problems which can result in a dental emergency within 12 months). Since I could not guarantee an emergency would NOT arise without getting a FMD, I had to inform the patient I could only sign it in the class 3 section of the form. He informed me that the form had to be filled out that day. I informed he that I could fill it out but only AFTER the necessary cleaning was performed. He seemed upset that I would not fill the form out with either a class 1 or class 2 dental clearance.  I can not and will not falsify records nor will I, in good conscience, call things vastly different that I see them. He left and stated there was nothing more I could do for him since, assumingly, I would not fill out the form the way he wanted me to. Dental radiographs were taken and an oral exam was completed, and they were submitted to his insurance company. These 2 items were the only services rendered.
