      This company falsely created documents with my signature under company R and E home healthcare and Sapphire home healthcare. The owner sent **** in my home to pretend to be physical therapist assistants and occupational therapist assistants and charged my insurance and created two invoices under both companies she used for payment from me. My medical record at this company was false and when asked to have records fixed and to be given a copy of medical records I was denied by owner and RN of company **** **********. I requested my medical record in writing on form given by office and was denied when given to **** ********** on 12/04/24 and I also emailed asking for medical records and corrections of visits. My insurance company was given false claims for services rendered to me for service before I was with the company, during the time I was receiving services and after being discharged. This company is the worst and if you are getting services from them check your invoices and verify the certification of the staff coming into your home. Please be careful when receiving services from this home healthcare because I never in a million years thought this would happen.

      Please review all charges and have the provider documentation for all services rendered. The week of R and E Healthcare Services has failed to correctly bill my son, ***************************** charges to BCBS and ********* February 17, 2022, R&E did not provide services for my son **************************** because the care provided **** was out of town. I believe R& E Healthcare has completed fraud with some of charges presented. The company was billing charges but deleting the claims and she refused to send me the claims back in January. I recently received a bill from her threatening to add interest to my charges but has failed to submit the claims to BCBS and ******** secondary insurance. I am very frustrated and stressed regarding this provider charges. I have never had any of his claims not paid and a provider making me responsible for $13,833.56 in charges. Please review and manage the claims. I have contacted his secondary insurance, ******** and informed them of the same issue. Charges should have been billed to insurance first before billing the patient. She didn't want me to submit the claims and deleted them. R and E Healthcare just recently send me the charges due to billing and deleting claims with BCBS which asked her for documentation. This has been very stressful and has caused me anxiety. This is not how a company should handle business for services. My son has autism and I have enough other issues to deal with than a provider that doesn't know how to properly bill charges for services rendered.

      I am fairly new with the company. I did send the letter with the hope of getting paid for the services that were rendered. It was not a threatening letter that was not my intention, just informing her of the outstanding balance with the hope that she would contact her insurance company for payment.

      R & E Healthcare didn't communicate effectively with the insurance and lack of insurance billing caused the claims to not pay. They informed my insurance that they shredded claims and did not follow up for patient with the primary insurance BCBS. R & E Healthcare is a ******** provider and my son has ******** as secondary insurance. They sent the letter to make it my responsibility to pay over $13,000 in charges. Due to lack of communication and business ethic I have contacted both insurance companies to let them know of the situation. At this point, my insurance should be contacted by the provider for payment and not send me a bill for services rendered. The proper channels was not followed and I am very stressed about receiving a huge medical for my son that have autism. This is the only provider that I have encountered this issue. Also, some charges that I am being billed for are questionable because no services were rendered.
