    - Date of transaction: ***** *** 2023 the contract/agreement was discussed and signed. **** ** 2023 the policy was distributed.
    - The funeral arrangements totaled to around $13,000+ in which he stated included taxes. He also offered to contact the life insurance company on my behalf and had me fill out a form with my information since I was the beneficiary. I came to find out that he had me sign a form that gave him and a third party that he never disclosed, the beneficiary rights. He also presented a Service Agreement that he never produced after the home appointment with an increased price that did not have my signature on it. Instead, it stated “Sign on Claim Form” and had an incorrect address on it.
    - $20,000 was paid to a third party: life insurance policy
    - Golden Gates committed to give the remaining balance of the policy: $5,025. I was promised that the check would be ready by **** **, 2023.
    - Have yet to receive the check as of ********* *** 2023. A Criminal Warrant for Theft has been issued on ********* ** 2023.
    - The company has made no efforts to resolve the issue. I emailed them on **** *** 2023 about the status of the check. After some confusion I was assured that the check was sent to my address. After some time, I still did not receive it and they did not follow up contrary to promises to do so.
    - I decided to get the help of a law firm through *********** to attempt to resolve the issue. The letter was sent at the beginning of August and waited two weeks for a response to that letter. He never responded to the law firm or me directly.
    - On Wednesday ****** **, 2023 around 7pm an email was sent from Golden Gates from Mr. B***’s email address to me and my daughter separately that indicated that the check was found in my brothers file. I was given two options to receive the check via a drop off at my home or through *****. I did not respond as it was apparent to me he was not trustworthy as I was informed on **** *** the check was sent on **** 18.
