    I started treatment at 100% Chiropractic in 2022. At the beginning of my treatment, I mistakenly called the Chiropractor, Ms ****** and she got very loud and reprimanded me in front of her employees and her patients. She said I work too hard to be called by my name by you. I believe that my situation has a lot to do with this incident. I think she is upset with me and disregards my requests. I decided to continue my treatment regardless of how I was treated. I completed my treatment at the exact location in 2024. I asked for my records and a recount of my visits. I received partial records and never received a recount of money spent at the location. They overcharged my credit card even after I stopped going for a while, causing the bank to shut down the account. They overdrawn my credit card by over 300 dollars. I would not have known if I did not ask. They would have kept my money. I applied for transitional disability due to my condition. DTA asked for a form to be filled out and signed by my treating physician. I provided the form to Dr ***** on July 12th of this year. I have not received a response from the doctor on the form nor my complete records and recount of funds as requested. She declined to answer countless requests. Her husband ******* who runs the place, told me she has been busy at home dealing with personal stuff and attending conferences. It has been well over 30 days, and I have not received a word from Dr ***** about my request. I do not owe the office any money. I want Dr ***** to give me my complete records, the signed form, and a recount of the cash spent at her office.
