    To Whom It May ********* have been trying to get my medical records sent from my old provider Care Point Medical (****************/formerly **************** office) to my current provider ********************** (********************). They use Chartfast for patients to request medical records. My first attempt happened on January 28, 2022. I physically visited Care Point Medical to get the release. I signed the form and they supposedly sent the request. I waited about four months because I realize these requests can take considerable time. I called my current provider ********************** to see if they had received anything to which the response was no. I went back to Care Point in June and they showed me where the submission went through. From that point on I have been going back and forth with this company to get my records. I have recently relocated which is the sole reason for me requesting the records in the first place. I decided to go through Chartfast personally to request my records and that was sometime in June 2022. The request was still pending some 20 days later. I filled out the release form online. I then contacted them through email on July 22, 2022, about my request and *********************** responded that I needed a wet signature to complete the request. I tried reaching out to her again to see if I could do the signature and fax it in and no response from her. I called yesterday and spoke to a representative and was told to email customer support, which I did (and have been doing) and to give them the ** of the request, then one of the support reps emailed the staff to assist me and I still have not heard anything back. I need my records so that they can be given to my new doctor. This company has poor reviews from what I have read. My next step is to contact the state's commission board to file a complaint.
