    After receiving service from this agency from 2021 to November 2023 for my daughter this agency failed to notify us that they were still receiving a payment for the government for services that my daughter and I did not receive. My daughter Medicaid was terminated in August because she was due to receive her disability benefits and she was going to start receiving her Medicare benefits, we got informed by our agency Source Care Management and they continued service as my daughter and I applied for Medicaid and was denied once again in October and we were told by Source Care that our case was terminated and the agency referred us to the Community Health Services to assist us in applying for a different Medicaid program. We applied for Medicaid thru Community Health Care in January and got connected with an agency and on March 05, 2024 we were denied due to the fact that Source Care Management was still receiving payments from Medicaid because we were approved for services thru company in August 2023 and we were not notified of this until now. I contacted Source Care Management which they denied that we were still in their system and were indeed still receiving payments from the government and I am my daughter’s caregiver and we have not received any services or payments from this agency since July 2023, and now we have to start the process all over and wait an additional 50 days for much needed assistance and resource.
