      To Whom It May Concern,I hope this letter finds you well. I am writing to formally lodge a complaint regarding the billing and insurance claim practices at *****From 9/20/2021 to 12/14/2021 I received medical services at your clinic and promptly paid the billed amount in full. I paid more than $10,000 out of pocket at that time. However, I was subsequently informed that my insurance company had not received the proper documentation to process the claim, which resulted in additional charges being requested from me. Despite my diligent attempts to resolve this issue with the billing department, the problem *********** my dismay, this unresolved issue has now escalated to the point where my account has been sent to collections, and I am being continually harassed with demands for an additional $1,800, which I do not owe.I kindly request that the billing department promptly and accurately submit the required information to my insurance company to rectify this matter. Additionally, I ask that any further requests for payment be ceased until this issue is resolved, and that I be reimbursed for any unwarranted charges incurred due to this oversight.The specific issues I encountered include:Lack of Proper Communication: The doctor did not adequately explain the procedure or the associated risks, leaving me unprepared and anxious.Improper Handling of the Procedure: During the **** the doctor appeared rushed and inattentive, which I believe contributed to the unsuccessful outcome.Inadequate *************** After the procedure, there was no proper follow-up or support provided, which added to my distress and confusion.Thank you for your prompt attention to this matter.Under the Fair Credit Reporting Act (FCRA), I am requesting that this inaccurate information be investigated and removed from my credit report.
