    The administrative office at a local funeral home suggested contacting Meadows Funeral Home to correct factual errors that appeared on a recent Georgia Death Certificate. The document was prepared on or around 7/17/23 for an immediate family member. The Informant, *********** Agent, P.O.A. and Immediate Cause were incorrectly entered on the copy obtained from *************. These errors extend from the actions of a family member with mental illness, and Meadows Funeral Home may not have been adequately informed. A request to make corrections to the certificate was mailed on September 25, 2024 but there has been no response from the office or mortuary. Please help us reach a more responsive agent at Meadows ****************** to complete this request without further delays.

    We received a letter from Mr. ****** H. ********* regarding a request to update his fathers (**** ****** ********) death certificate. His father passed away on July 17, 2023.

    He wants box 13a (Informants Name) updated from his sister (***** *********) to himself. The informant is usually the next of kin that provides information for the demographic information for the death certificate.Also, he wants box 32a (Immediate Cause) of death updated to a different cause.

    The letter we received in September 2024, indicated he was the Informant, *********** Agent, and P.O.A. for his father. However, we have another document from his sister indicating she was the *********** Agent for her father. His documentation was dated January 25, 2020, his sisters documentation was dated September 5, 2020.

    He is claiming her documentation is fraudulent but we have not received any certified documentation providing evidence of this. He included a letter from the State of ************************************* regarding possible abuse and neglect of an elder. However, the document only references a report of possible abuse and neglect of an elder there was information provided regarding the findings of the report or date in which the report was made.

    Since we do not have certified documentation from an official indicating his sisters (***** *********) documentation is fraudulent for his father, **** ****** *********, we cannot amend box 13a. If he can provide the necessary documentation, we can work with *************************** to amend the death certificate.

    Also, a funeral home cannot update box 32a because this information is provided by the certifying physician. He would need to contact the doctor and discuss why the cause of death needs to be updated. If the doctor agrees, our funeral home would work with *************************** to amend the death certificate.

    I am rejecting this response because: the informant-in-fact ****** ********* has co-signed specific terms of agreement with the deceased (victim) stating that he would not allow others to engage in any illegal activities to the extent that (the agent) has the power to stop such activities. Any potential compromise to these terms due to the actions of others would be met with legal action. The agent has acknowledged and endorsed in writing that he has read the terms of agreement, understood it, agreed to it, and has been given a copy. Under these terms, the agent agrees to provide and maintain the patient's health directive, personal  accounting, monitoring utility bills.. home security and scheduled maintenance, grocery shopping.. routine service to vehicle.. co-execute estate plans.. conduct business on the premises.. and to maintain the home as his permanent address. Agent also agreed to park vehicles appropriately in designated areas only and to take responsibility for where visitors park. Agent shall not assign terms of this agreement to anyone else without first obtaining written permission. These terms, initiated on March 12, 2006, by and between **** ********* and ****** ********* (Agent), were mandatory for occupancy under the (above) terms and conditions.

    Concerning ***** *********' history of substance abuse and pathological behavior the hospital social workers cited a serious psychiatric disorder. Furthermore, the victim had a lifelong neuro-developmental disability with no functional capacity to manage her actions except for his assigned agent ****** *********. The *********** Advance Directive dated 9/5/20 was not introduced in accord with the victim's personal wishes and this and other unwanted or fraudulent alteration was quickly reported to authorities. These authorities include county police, ************************* and a probate judge. The deceased' agent and attorney-in-fact requests that Meadows ****************** amend box 13a.  Meadows ****************** is also requested to update box 32a because the physician cannot be reached. The funeral home is expected to work with *************************** to amend the death certificate. ?
