      Was given incorrect information for Provider and facility. When trying to submit claim to my insurance, the company claims they had a mixup with Provider NPI, when I called a new number was given. Now the billing-facility NPI number is incorrect and the corporate office states they gave the rightful information. Patients are left with such a big bill due to negligence. How can you mix up Provider NPI’s? This is peoples lives and health information.
