    Diagnostic Laboratory Services, Inc. ("***") should be fined for their unethical billing practices. I've had two bad experiences with their billing. I have an excellent credit rating and have paid all of my bills on time for my entire life. In the first incident, *** referred my account to a collection agency. Upon receiving the notice from the collection agency, I contacted *** to ask for a copy of the bill. When I reviewed the bill, *** had sent it to the wrong address (I only received the collection notice because my neighbor gave it to me). 6 months later, *** sent my "delinquent" account to a collection agency again. I again contacted *** and they mailed me a copy of my "delinquent" bill which was dated October 2019, three years ago, which was sent to an address I haven't lived at for over two years. Why did they not address this "delinquent" bill during their first mishap? Why wasn't it sent to my correct address? Why didn't the *** representative raise this alleged second bill when we were discussing the first one? In addition, in mailing me a copy of my "delinquent" bill, they included an invoice for someone else which listed all the testing that this particular person had undergone. Surely, *** has violated some type of regulation or law.

    It happened at my doc's office, but the phlebotomist was from DLS. I was there for generic carrier testing, and the phlebotomist thought I was there for HIV test. I told her what I was there for. She seemed very confused, while she had the paper work plus the testing kit that says generic carrier testing on her hand! Eventually she figured it out. After she put the needle into my arm, she grabbed the testing kit several times trying to read the instruction. She looked very nervous. When she was done, she looked at the vial and said the vial was not full, and she was not sure if the test would be accepted...The nightmare started after. I had a giant dark bruise on my arm and it was very sore. It actually got to the worst after 7-8 days. Lots of people asked me what happened. It finally went away after about a month. I would like to attach pictures if I could. I emailed the organization and someone did call me to follow up and apologize. It seems like this person is new and there is lack of training and supervision there.
