      Oahu Emergency Phys Services LLC received and cashed my check payment of $12.00 for physician services rendered on 9/27/2023 as below:Provider: *************** MD Procedure ***** Patient ************* Account number ************** See the 2 attached image files showing check number 2725 was cashed and deposited by them on 4/29/2024.I called their customer service at ************ on 5/22/2024, where I explained that the check I had sent for $12.00 was already cashed and deposited by them. Also I explained I continue to receive statements showing an outstanding balance and text messages to my personal phone that the matter will be referred to a collection agency.The customer service *** said she did not have the ability to view my account payment history at all and was not able to verify my payment was made.I recently received a letter from a debt collection service named ****** stating that I still owe the $12.00 copayment.This referral to a debt collection company will unduly affect my personal credit rating if it hasn't already been affected.Oahu Emergency Phys Services must immediately clear my account balance, contact and remove the debt collection actions by ****** and rectify their billing system to not keep outstanding balances following bill payments already received.I expect a follow up letter from them to indicate the above steps have been taken.
