      This patient has UHC commercial with $750.00 deductible and 20 % co insurance.  Patient has had cataract surgery. The 6/27/23 bill is the 20% *** insurance of $223.95 from this surgery.  The date of service 7/12/23 is a Post-op refraction that was done that the patient would not pay at time of service,  per notes in the system.  This is the non-covered charge per insurance EOB. We called patient on 6/21/23 to review benefits and left a message for him to call us back. It does not look like he ever did. The picture of the bill for $50.00 is an office *** pay that patient did pay.   

      The patient asked us to bill to crime victims in ******** as his secondary insurance. When we called to verify, we were told the case was closed and that he would have to call and reopen if needed.

      There are no notes from patient accounts that the patient has ever called to inquire on this balance. We would have been happy to explain all charges, if he had contacted us. If he has further questions, please have him call our patient accounts supervisor, ******* at ************.
