    I was exiting an abusive marriage and my children, ****** (age 2-3 at the time through it all), and ***** (almost 0-1 at the time through it all) were victims to my now-ex-husbands behavior. I brought them to this facility on multiple occasions throughout the proceedings over the year until finalization, for neglect. There was a history of sexual abuse between my exs children from a prior marriage, and they now resided with him in his home with our children. I notified the facility of such as it is incredibly important information to communicate in situations such as this. I REPEATEDLY stated to every practitioner that examined my children that while this history is there, I don't think that it came from my ex. And I have this on voice recording from one of our first appointments as one example.However, when they were asked to submit records to the courts, in the unrestricted notes that legal professionals see while customers are not given these notes in their version of their records, the practitioners repeatedly stated that I told them that I DID think it was my ex who was responsible for this type of behavior. This was given to a court appointed party (***************************) that was called in to help determine custody. And was noted in her report and weighed heavily in her decision.I tried speaking to the practice about this but they would not allow me to do so in a passive manner. So I moved my children to another practice and left their records because of this serious discrepancy.I just moved and have records in paper form as well as the voice note saved on my phone that I can email. Please forgive me here, but can I submit what I have over email so I can get them scanned in and compiled?

    I'm not quite sure I understand the complaint. We keep record of what each parent says either in visit or on the phone and I'm not sure what record it is that was seen by the court that she is deeming incorrect. If she has something that she would like to submit to our office that needs to be heard, I will hear it. She hasn't contacted me so I haven't been given the chance to help her. There is or was a very contentious divorce between her and her husband. We have documented conversations from both of them and only document what they tell us.
