    I was previously a patient. I had referred a friend to be seen by the same office. The office while treating this referral hired this person to handle their front desk and paperwork, including my documents. This is a breach of confidentiality and an unethical privacy violation. Within the last month, I have received statements from the company showing that fees have been added to my account for the following dates; 12/28/22, 10/17/22, 11/02/2022, 11/16/2022, and 12/14/2022. The current balance on the account is $80.05. This is an unethical billing practice to back date fees for almost a year prior, with no explanation to what they are for aside from a fee adjustment. I have called the company twice and no one answers, we have left messages but never receive a call back. I am requesting the fees to be removed and the previous balance of $0 to be reinstated due to the companys unethical negligence.

    I attended sessions with **************************, *** ***** EMDR-T, RS in 2021 through Jan 2022. During this time all sessions were submitted to my insurance but i was never provided a "Consolidated Summary of Services" within 60 days of services as required by the Idaho Patient ACT: *******************************************************. Metamorphosis ******************************* Center of Idaho sent me a bill in July 2023, over ******************************* Jan 2022. This billing included charges from over 2 years ago. When I asked why is am not getting a bill, their response via email was : "the office had been short staffed since COVID and were unable to enter the EOBs from insurance." This excuse is not covered within the ***** timeframes. Since I had not received a Consolidated Summary of Services or a bill or final bill from ************************** or Metamorphosis ******************************* Center of Idaho via mail or email that they had on file, my thought process was I did not owe anything. I feel this is an abuse of me as the patient and should not owe anything as I am not responsible for them not following ***** House Bill 515
