    I was a client of the Fox Valley *********** center until 2021, when my primary care ******* ********************************** retired. I felt that my account was paid off until I received a text message on Friday evening on 5/24/2024 after normal business hours, claiming that I owed $50.00. I called the clinic and the associate informed me that she received many calls about this situation. I also received a collection call on Memorial Day. On 6/2/24, I received an email from ********** claiming that the bill is legitimate and he requested a payment and sent me an EOB that was dated 6/2/2024 and records showed that my last payment was on 5/31/2021 in the amount of $25.00. If I remember correctly, I had a payment arrangement and that was my final payment. I no longer even have the insurance coverage that I had at the time. I received another text message this morning on 7/1/2024 but I just reported the text as junk and blocked the sender. I advised ********** that I am not going to pay the bill due to the length of time that they requested a payment and advised him to send the bill to collections. The fact that I received a text message on a Friday evening instead of receiving a paper bill made this more suspicious. Getting a collection call on Memorial Day upset me even further.

    Attached is my signed HIPAA form.  Thank you for your time in handling this legitimate complaint.  I was told that my balance was paid in full in 2021 and now they are try to collect an additional $50.00.

    We have sent **************** statements in the past in 2021, and he has not paid the amount due to us.

    In his own explanation we have provided him proof from his insurance company and a statement emailed to him as well to justify the charge.

    The outstanding balance is his responsibility per his insurance.

    I am rejecting this response because:
    I still disagree that I owe the $50.00.  I had an agreement to pay $25.00 per month until the balance was paid in full.  My last payment was on 7/30/2021.  I made the payments in good faith and even deleted Fox Valley Immediate Care from my online bill pay service through my credit union.  I do not intend to visit this healthcare provider again due to what I consider to be shady collection practices.  The fact that many patients were calling in the same day that I did when I received the text message shows that I am not the only one who felt that it was a scam.  Even the workers that day told me not to pay anything online and another worker questioned why it took so long to be contacted.  
     
    If I owed the $50.00, why did it take the clinic three years to collect on the balance owed?  As far as the *** is concerned, it didn't show the actual date of service, it showed the date of 6/2/2024. 
     
    Per Quicken, I paid $25.00 on 5/26/21, 6/30/21, and 7/30/21 per Quicken since I owed the clinic $75.00.  Perhaps I can call my online bill pay service to see if they have records of those checks being cashed.  The check numbers were 7232, 7233, and 7234.  
     
    I do not intend to pay this bill.  Thank you for your time in handling my inquiry.  

    One more thing, I think I figured out what the error on their part was.  They indicated on their statement that my last payment was on 5/31/21, so that was the payment that I made on 5/26/21.  They either mistakenly or intentionally did not credit my payments on 6/30/21 and 7/30/21.  I did call my bill pay service for the payment records but this will take longer since I deleted Fox Valley Immediate Care from my list in 2021.  I did not hear from this provider again until I received a collections text message from them on or around 5/24/2024.  
     
    Attached is a copy of my Quicken ledger showing the payments that I made to Fox Valley Immediate Care in 2021.
     
    Thank you once again for your time in this matter.  I would also like to make sure that other current or former patients aren't taken advantage of by this healthcare provider.
