    I am rejecting this response because: nothing from the dentist of ever showed any amount charged either at the point treatment or after. When asked how much the procedures would cost all that was presented was a treatment plan with actual numbers. Even if the numbers are an estimate they should be not be over 1000% without notification of change in price. I have presented actual numbers as evidence and no where did I sign or agree on the amount being charged.

    Michael ********0+ before being able to be seen again. First time I heard anything about this. So I told them that I dont think thats correct and they said the lady who handles that was out that day for personal reasons and she would call me back the next day. She did and left a message telling that the bill was correct and if I had any issues to contact my insurance provider. In which I did. They informed me that rates change but the healthcare provider has those changes at the beginning of every year. Now the dentist office has sent this to collections which is negatively effecting my credit score. I would like the dentist office to remove this bill. They set a price in which I agreed and kept asking for more as treatments continued. I would not have had it all done so soon if I had the actually price first. I do understand if it went up a little but the original amount went from $43 to over $700 that I was to pay. Not one time was I informed that the charges were going to increase.

Our office is committed to providing accurate estimates while acknowledging that our information relies on the details provided by the patient's insurance plan. It is important to note that the treatment plans we offer are solely estimates, as outlined in the financial agreement signed by ********************. We apologize for any inconvenience this may cause the patient.The remaining balance is still due.
    Financial Agreement:
    Please remember your insurance policy is a contract between you and your insurance company. We are not a party to that contract. As a courtesy to you, our office provides certain services, including a pre-treatment estimate which we send to the insurance company at your request. It is physically impossible for us to have knowledge and keep track of every aspect of your insurance. It is up to you to contact your insurance company and inquire as to what benefits your employer has purchased for you. If you have any questions concerning the pre-treatment estimate and/or fees for service, it is your responsibility to have these answered prior to treatment to minimize any confusion on your behalf. Please be aware some or perhaps all of the services provided may or may not be covered by your insurance policy. Any balance is your responsibility whether or not your insurance company pays any portion. Any invoice or receipt issued by Michael Smith DMD LLC is a nonbinding estimate only, and additional charges may apply depending upon actual amounts remitted by your insurance company for services rendered.
