    I had a minor issue with my left ankle. Before making the appointment with ********************************* I was told at his office that the cost for the appointment was only $125. It was necessary to pay that amount from my pocket before the appointment.If there was a need for a injection, the charge would increase.I had the appointment with ********************************* on February 14, at his office at ********************************************. I paid at the $125 with a check before the appointment.Dr. Smith check only my ankle. I did not receive an injection or requested other services.He recommended a special boot (AFO Molded).I asked the nurse for the price of the boot. She said that the cost of the boot was covered by the payment of $125.. She did not mention anything related to my medical insurance.Based on the information they provided I consider that I paid in full the service that we agreed.I never used the boot. It was not necessary and is completely new.On March 5, they sent a statement. The statement indicated:1) that price of the appointment was $185 without any explanation, and I had paid only part of that of the bill ($ 125).2)that the price of boot is $650; my insurance had paid $278 3)that I owe $431 The ** for this bill is CSD 28660.This statement is attached..On April 3, I went to *********************************'s office. I explained the situation to her secretary and she took notes on the computer. She went to talk the manager. The manager did not talk to me that day. The secretary mentioned that the manager was going to call me. However, the manager has never called me. I mentioned the secretary that I could return the boot because it is new. The secretary did not accept the boot.I want to pay what we agreed, and for the service that we agreed. This amount was $125 I paid it in advance. I should not paid what we did not agree, $431.I consider that this type of medical service and this type medical bill is an abuse from ********************************* to me and to any future patients.
