      Office needed prior approval (PA) before the procedure. I asked if PA was received as date was 08/13/24; fiscal year started 07/01/24. They said yes so I agreed to proceedure. Surgery planned for 10/24/24 needed PA as well. **** letter of 100% coverage from ********* saying insurance was contacted for surgery also. Later office admitted they never called insurance just guessed based on the file. Insurance stated no record of any contact from the office and ** is out of network as of 07/01/24 so services are not covered. Office will not return calls to address the situation. New ** says surgery is not even needed.

      Patient as in to see Physician on 08/13/224 or **************** and deviated septum. Physician performed a nasal endoscopy to view sinus issues, this is billed as a surgery but is not a surgical procedure.   Patient signed a consent for nasal endoscopy that fully explains the procedure and how this is billed to insurance. Patient has had this procedure in the past.   Physician is in network with patients' insurance and has been for multiple years.  Claim processed initially as out of network, then reprocessed on 01/17/2025 as in network with payment on procedure. Billed $475.00, insurance allowed $420.63 , contractual adjustment was $54.37 .  We contacted insurance prior to second processing and insurance reassured us that we are in network. On 1/6/2025 patient paid $110.00 which now is reflected as a credit on the account.   We will credit the patient back for this payment,
