    on 12/13/22 from Lake county Behavioral health for serve depression I had recently recovered from covid and tested Negative on 12/10/22 When I first arrived there I had my own medications sent with me so they could manage them. They claimed they didn't have any records sent so I offered to give them through my patients portal which they declined them. Three days later around 9 am during med pass I started having an asthma attack which the cna put a monitor on me and it's was confirmed my oxygen was at 83%. There was a another worker watching the scene; my rescue I inhaler was in sight but the nurse ***** made me wait until her med pass which was around 14 mins then I was giving my inhaler. The worker who witnessed the incident informed me this is not right and I should file a grievance which I've done. days afterwards I woke up with severe chest pains and came to the same nurse on that floor to address the concerns the cna took vitals which my ** was 177/111 and my heat Rate was 145 hypertension stage 3 which she deined sending me to the er. My family also called with concerns and was hung up on and one cousin cursed. This went on for two days I call 911 but they told me staff would have to make the call after me calling 7 times In two days they came out and I was taken to St ******* in ********. At the time and still today I always had all my rights to make my own decisions which is also on the contract I signed with Albany Care. The same nurse called them and lied and stated that I was delusional and nothing medical is wrong. I told the er Doctor that I had chest pains for 3 days my vision is blurry and I feel like I might faint. They treated until the call and the atmosphere changed. I asked them why haven't I got my consent for treatment and my hippa form with no answer. they did light check up and I had to be psych cleared which I was the medical issue wasn't resolved so I begged to go back and they sent back where I was assaulted and my phone was broken
