    On October 31, 2022, I had an appointment with ***************************, Lic. #**-078950, at **************************************************************************************. Although, additional receipts to my transactions showed an address as ************************* in Olympia H3333303334343432**31H, **. *****, the first address, is the one I went to. After having spent 4 hours at **************** office, which included a thorough eye exam by **************** Staff and **************, I received a prescription for glasses, that included, one for reading and computer purposes and another for driving purposes. I purchased 2 pairs of glasses that I urgently needed, with that prescription. After receiving my glasses, I tried them for several months, and I was unable to see clearly. After being so frustrated with not being able to do my work, which required a significant amount of computer work, I decided to schedule a separate appointment with another Opthomologist, to get a second opinion regarding my vision. I took **************** Prescription and showed it to **************, of Primary Eyecare Professionals, located at ****************************************************************************************. He could not believe that the prescription that Olympia H3333303334343432**31H Eyecare, **** gave me was extremely incorrect. ************** corrected the prescription, and I took it to ************** on June 23, 2023, I asked her to please re-do my glasses with the corrected prescription. ************** couldn't believe how incorrect ************** discovered my prescription to be. Per **************** request, I left my 2 pairs of glasses with her, because she said she was going to call ************** in order to figure out where her mistake was. I waited over two weeks and never got a reply from **************, as a result, I decided to pick up my glasses, took them to **************, and had the lenses on both frames replaced with the corrected prescription. Now, I can see perfectly and I am very content about this. The only drawback now, is that my two pairs of glasses, purchased with the incorrect prescription, cost me $ ******. I incurred an additional cost of $423.28 after replacing the lenses on both pairs due to **************** mal-practice. I kindly ask that you please help me with this situation as I feel as though ************** was not only negligent and careless in her diagnosis of my eyes, but she also didnt care about me needing my glasses and taking so long in trying to remedy my situation. ************** never showed any interest in me not being able to see clearly and did not want to replace my lenses as she tried to brush me off. I ask that I be reimbursed a total of $423.28 by **************** office for the financial loss and hardship due to her negligence and mistake.

    I am refunding her in the amount of $423.28.    It is a check issued on US Bank with a check number of 0930.  The exam was done on 5/23/22 by a doctor from somewhere else.  July 2023 is when she came in with the glasses.  I will have her come in to pick up the check and return the glasses.
