      In June of last year I had three implants put in. I told Dr. ********************** that I wanted them smooth and without points. FYI, all my teeth are capped except three on the bottom. I also had a tooth that he had to do a root canal on and when I went back to have the cap put on I told him that it was still throbbing and he said that was not a problem and put the cap on....a few months later I came back in severe pain and he had to remove the cap and entire root and I had to have an implant. I told him that I wanted this tooth smooth and without any points. When I returned to put the implant on it was a nightmare he did not listen to me and it took me coming back at least six or more times for him to correct this tooth. Now lets get back to the original three implants he put in...I most have gone at least twelve times to get them corrected and two weeks ago I went back again because I was still having problems and he never listens to my complaints always has an attitude why not he got his 11k already from and actually makes the problem worse...now I am at the point because the teeth were never made probably to begin with that he can not make any more adjustments and I am in discomfort every day. I can not live like this for the rest of my life I have never had any problems with caps or bridges like I said my whole mouth except four bottom teeth are capped. I wake up and go through the day with pain and discomfort and to be honest just want to sleep so I don't feel the pain.

      The patient first came to our office in June 2021 and had 3 implants placed. We placed a fourth implant adjacent to the other three on May 2022 and had the final crown placed September 2022. From the placement of the initial final crowns in July 2021 up until when we last saw the patient in March 2023, they have come in a total of 13 times for an adjustment of sharp spots on their crowns. Dr. ********************** has never had to make so many adjustments to a crown as he has had to do with this patient.


       During each of these visits, The Dental Assistants and Dr. ********************** would patiently cater to the needs of the patient. Dr. ********************** has tried to meticulously polish out the sharp spots that the patient points to on the crowns. The patient has recently tried to help pinpoint the source of discomfort by drawing the occlusal plane of the crown along with the locations of discomfort on a piece of paper. After using our polishing burs, we would repeatedly ask the patient to let us know if we had properly adjusted the crown to their liking to make sure that Dr. ********************** did not miss anything. At the end of each visit, the patient would express their approval of the adjustments made. If they still had complaints about sharpness as they were leaving the office, we were not made aware of it. Sometimes as soon as 1 week after the last appointment, the patient would call back with the same complaint about sharp or rough spots on the crown. After the last appointment we had in March 2023, the patient was informed that due to excessive polishing,further adjustments may compromise the integrity of the crown.


      There is no evidence to suggest that the patients pain is due to a faulty implant. Multiple x-rays and CBCTs indicate that the implants have successfully fused to bone with no mechanical failure of the implants,radiolucent regions, or intrusions in the patients anatomy. Clinical signs indicate that the implants are solid upon palpation and are negative for peri-implantitis. Because the implants have fused to bone, it is highly unadvisable to remove the implants.


      We explained to the patient that the crowns cannot be perfectly spherical or flat. Especially with molars, the crowns need to have centric stops to form a proper biting surface with the opposing teeth. The occlusal surface of a molar or premolar crown therefore cannot be perfectly flat if it is to function properly.


      During a visit in November 2022, the patient noted that their dental pain is often associated with work stress. The patients dental pain may possibly therefore not be caused by sharp spots on the tooth, but rather from clenching/grinding. We recommended that the patient purchases a mouthguard to protect their teeth from clenching/grinding at night. However, the patient has continued to express their discomfort with the sharpness of the implant crowns after receiving the mouthguard. If the origin of the patients dental pain is from clenching/grinding, then we would recommend receiving Botox treatment of the masseter muscles.
