      I had an appointment to have my toe nails clipped and get a diabetes evaluation at the same time. Upon entering the Fox Valley Orthopedics office in ********, I paid the $40.00 co-pay with my **************** credit card. When I was called, I entered a room, sat down and was questioned by an associate as to my visit. I told her I needed my toe nailed trimmed and an evaluation relating to type 2 diabetes. I was told that they do not perform that service. At that time, I told them that was my main concern and that I wanted to cancel my appointment and be given a full refund of $40.00 and have it credited to my **************** Card. I waited nearly 20 minutes while their staff tried to credit this transaction. I was given a copy of an E-Payment Activity (see File SCN2A) and was told that everything was OK. I waited several days, called **************** and was told that the company, Fox Valley Orthopedics needed to send them a credit for the $40.00. On July 29, 2024 I called Fox Valley and spoke to **** who knew about the situation at hand and said I should wait several days and call back if I needed to. I waited and checked Amex on August 4, 2024 and found no credit had been sent to them. I called **** back again on August 6, 2024 and told that nothing had changed and no credit was issued. She transferred me to ********* in the *************************** I was then put in *********'s voice mail system to which I left a detailed message and waited several hours without a returned call. Although **** was very nice and appeared to be helpful, I'm still without a resolution. All I want is my co-pay of $40.00 returned to me, either as a credit though **************** or a check made out to me so I can cover the amount I had paid to the credit card company.

      I cannot get someone call me back...they stopped sending invoices to ** on two accounts with them where we have payment plans in place. I reached out to *********************, who has been helpful in the past, and she has advised I would be hearing from a ********, I assume in billing. I have sent them a detailed email on what I need, all I am trying to do is confirm that the balance I show we still owe agrees with what they show. I do not know that, since they have not been sending invoices. The accounts involved are Patient ID ****** (last statement received 2/28/23) and I show we owe $952.14. The other account is ****** (Portal C), and we have not received a statement since 2/9/23. On this account I show we owe $293.95. This company is in the middle of a billing transition and it is creating nightmares with all separate account numbers. I want to avoid a debacle and all I ask is that someone call me back at ************.
