Thank you for allowing us to respond to the complain made by *********************.

********************* was originally seen at our office, Dental Arts of Huntley on 12/15/2023. A no charge consult was done and a panoramic radiograph was taken that was billed to insurance. After discussing with *****, a Treatment plan was created that included her treatment to be completed in 2 phases. Everything was explained to her in detail and she signed both phases treatment plan. Please see Treatment Plans attached.

Phase 1- Interim Maxillary complete denture, Extractions of Maxillary teeth and 4 Endosteal Implants. Cost of phase 1 was $5816.80. Please see the Treatment plan attached. This was suppose to be done in Jan 2024.

Phase 2- Precision attachments over Implants and a final Complete maxillary over denture. This was suppose to be done 2-3 months after phase 1 is completed. We even wrote it on the treatment plan that it will be done after March 2024.

Due to waiting period on her dental insurance policy, Her insurance only covers extractions. We have received only $278.40 for her extractions from insurance. Her claim that we received $1080.00 from Insurance is not correct. Please see Estimate of **** and billing statement attached.

On 12/21/2023, She paid us $500 + $5316.80 as deposit for phase 1 treatment. $500 for the interim denture, $316.80 for extractions and $5000 for her dental implants. Please see credit card receipts attached.

On 01/26/2024, Phase 1 was successfully completed.

On 02/06/2024, A post op visited was completed and ***** was healing great.

On 04/05/2024, ***** came in to start phase 2 of treatment plan. She had healed really well and cost of phase 2 treatment plan was shared with her again. She did not want to pay any more for her treatment and left the office.

There is no case here for a refund or us taking advantage of her. Everything was explained to her and she signed all documents. She paid us for phase 1 and phase has been successfully completed.

Hope this explanation helps. Please feel free to reach out to us at ********************************************************** or ********** for any additional questions or concerns.

    Thank you
