    I requested all medical records and clinical notes and did not receive them. ******************** did not contact me for my therapy because they did not accept IA ***** *********** never looked at my doctor ***** to know what I needed. Requested they send my ***** to Trinity and was sent to different entity. Missing notes from visit 11.29.2022 there was a woman in that room writing notes that day and I was told the *** of corporation was waiting in break room for a followup with my doctor. In addition there should have been notations about the *********** supervisor being involved and contacted for an explanation on why ***** picked up my physical therapy ***** properly ready doctor instructions and dropped the ball here. Please help I need all my information for my second opinion asap thank you

    Thank you ******************** for helping consumers. *** ordered and I paid for xrays, MRI's and a test using nuclear markers for an ongoing leg issue starting before Covid in 2018 and ended with me quit working October '21 because I was bedridden. Couldn't find a solution. Now I have no insurance or income. Sold home in ******, moved to ******** 6/1 near adult daughter in senior income based housing. Went to *** in person late May, 2022 and signed, with ink, the form for release of medical records, similar to your's here electronic permission but they use a paper form. Was told they'd put it in my file, when you have a Dr in ** call us and we can send your test's. Called in July '22 with Neurologist info with University of ******** Health. They told me they lost the form!!!! Sorry, end of conversation. Asked for supervisor, *****, told her I have no printer, nor access to one, she said would email electronic form, sign, return....I confirmed I could sign electronically and email back? Yes. ***** said if my file is too big they won't "fax"?Ok, I don't work there & have no idea how records are transferred. Confirmed my email and crickets. Never got email. Called and reached *****************************, again, and again, cuts me off, nothing she can do...never any offer of help. ****** emailed me paper form, reply, again, can't print, etc and again....crickets. Emailed back now twice, begging for help and silence. I need ALL history/test results sent to *********************************************, ******************************************, ****************************************************************** ************. ******************** uses an electronic form, not ***? It's 2022, we've landed on mars! Why would they send me a paper form that they ALREADY LOST? I paid a lot of $ out of pocket for a lot of tests and they never could figure out what was wrong. I have no income, I have no insurance and can't afford to have expensive, like REALLY, expensive tests repeated. I DIDN'T LOSE THE FORM! TY ******************** for helping us who have no voice.
