    Dr. ************************* is employed by " From Pain To Wellness. Without going into any detail please let it be known that the psychologist is cognitive behavioral issues & violated my civil rights and breach her care of duty per legal counsel and I am off & on struggling now with ( PTSD)- post traumatic stress disorder/depression/worthlessness and I'm a physically & mentally disabled by the federal government. Over the entire year & prior leading up from my last appointment with **************** on October 7th 2022 at 10am. We had a very promising DR/Patient ************ and discussed my anxieties/Bi- Polar disorder & most importantly rebuilding my ************ with my only Daughter (Child) now 21 years old. Our last appointment i mentioned to her that I was slandered on the ***************** said " Let me take a quick look:. Within a long moment she stated I'm now ending this ********** hung up on me. I called back the office to find out she was working from home & not the office. Lastly she never contacted me for our November 9th 2022 11:00 am appointment. I was never notified by a call or mail about what I should do next & became extremely distressed and felt a sense of hopelessness. I really lost hope in rebuilding my ************ with my daughter and eventually moved out of state. I felt leaving ******** would get me sober as I severally struggled with day in & day out while constantly drinking 750ml bottles of Vodka daily. As a born again ******************** had to increase my faith in *********************** & as of today its been 72 days since I touched a drop of any alcoholic beverage. While I firmly believed that my life would end in suicide if I didn't make it to the age of 53. On January 15th **** I finally made it to 53 yrs old & spent my bday with family in *******. The scariest part of my story is I have never once met her in person in which is possibly another Federal or state violation & finally I googled her image so I can put to rest the demons that at times haunt me.
