    Medlab is offering me a job to work from home by entering data. ********************* is my contact person. She was unable to email me the background check authorization form, Offer letter, employee direct deposit enrollment form which states at the bottom that not only can they enter funds they also can take them out for erroneous reasons, then on the job details letter it has the name ********************** **** Offering $18.00 per hour to train and then $28 per hour once trained. Another concerns that there are spelling errors on the forms. Unfortunately I have supplied them with all of my info except my Social Security number. Is this a scam. I am very skepital about this and wondering if they are trying to steal my identity.

    As ******************** too received 4 COVID boxes for testing & they used my ******** # to submit a$96.00 claim which was paid by ******** no question! Don't answer phones or voice messages called twice no response. No idea why I got them. How do I file a fraud claim to get the amount credited as an adjustment to my ******** deductible which I pay a monthly ******** premium on..so I'm actually paying for it! Please take action on this matter and give them the proper rating! I will also be notifying ********.

    This company fraudulently submitted a claim under our ******** number and secondary insurance. They submitted a claim for 4 Covid Home tests, the same ones we received for FREE two years ago. They were paid well over. $300 for these. They dont answer their phone and no one calls back when you leave a message.

    We sent him an email yesterday and he didn't response.
