      A breast implant exchange was scheduled and completed by the facility. Payment for the procedure was completed in full on 1/29/2025; $8000. The manufacturer of the implants (Mentor) offers a warranty on implant ruptures that occur within 10 years of initial placement - I brought this up to the patient care coordinator, ******, at my initial consultation (1/20/2025). Mentor pays the cost of the replacement breast implants to the provider so that they are technically "free" to the patient. On 2/14/2025 I received confirmation via text from ****** that the breast implants were covered under warranty and therefore provided me with a $1200 credit. I asked to be refunded instead and she said it was possible, but was a lengthy process (6 weeks). Since then I've called various times to inquire about the status of the refund and have not been connected to anyone that could provide answers. I was told that I would be refunded and it was noted in my file, but only ****** ***** could issue the refund. At my 6 week post-op visit (4/23/25), I was informed ****** was no longer employed by the facility and the refund was not communicated to the billing department and ****** ***** would be reaching out to me; she never has. I left a voicemail May 15th & received a refund of only $1000 - again, I have not been contacted by anyone to confirm the refund or banking information. I opened the initial claim with Mentor, informed the facility of my intention to make use of their warranty, and received the correct Mentor REPLACEMENT implants - the agreement was to be reimbursed for the cost of the implants. I've received $1,000 of the $1,200 and have not been able to successfully get ahold of anyone to discuss this.

      I am a longtime patient of PLASTIC SURGEONS OF AKRON and Dr. ********* I ordered another 4 vials of Semiglutide WITHOUT B12 on 4/16. *** ******** the nurse practitioner at Plastic Surgeons of Akron, ordered it with the B12 with the compounder and I received an incorrect order. I opened the package to double check on the contents and saw it had been ordered wrong. I texted *** to let her know with no response. I then went into the office with the package and *** was not working that day. They tried to get ahold of *** also with no response. I was assured it would be addressed the following Tuesday when *** returned to work. I did not hear back from the office so I texted *** again to please call me asap. Once again no response. Her arrogance and in not addressing the problem is a direct reflection on Pryor Health and Plastic Surgeons of Akron. I have $2,400 worth of Semiglutide I cannot use and want to return the products and receive a refund promptly. I have a serious medical complaint I also need to address with corporate and Dr. ******************** on her handling of patient instructions in administering the product. I see a serious liability with patients in regards to *** as a practitioner on the Semiglutide. I did address that with Dr. ******** shortly after I began using Semiglutide in May of 2024. I feel corporate also needs to be aware of this. I also have text messages I have with *** that I have saved. She has also sent out the wrong semiglutide to other patients I know.

      Dr. ******************** at Pryor Health also known as Transformations Plastic Surgery, performed surgery on me on 2/23/2022. The care received on the day of surgery was poor, pain was not managed, I was pushed out of the clinic before being in an acceptable state, and information regarding my care was not relayed to my caretaker before discharge. ******* was expressed with ***** the ************ Coordinator when she called at 5 PM that day and also at my 24 hour follow up on 2/24.2022. Concerns regarding appearance of surgical site were also noted this day and were dismissed. In requesting my medical record to provide to my insurance company to file a claim for the surgical costs, I received a record that contained numerous inaccuracies. Some examples of these are, incorrect surgical sites being operated on, that implants were ruptured, incorrect amounts noted of what was transferred into my body. These were addressed with the provider and staff at my one month check up (which was pushed to 6 weeks due to the provider being on vacation), and I was told that they would be corrected. I also requested the video file that was promised and was told they would have to determine how to send the file that would be 2 hours long. After many phone calls and meetings, I received a 1 minute long video of after surgery, which provided proof that the implants were not removed as noted in the medical record that I had received, and when addressed with the doctor was told this was semantics. I still have yet to receive a copy of a correct medical record for the procedure performed on my with Pryor Health. There were no check *** performed on two of the sites until I requested this at my 3 month check up.I have sent 2 certified letters to Dr. ********************s office in the past 6 weeks with no response. They have now left me with no recourse to even attempt to recoup any of the cost of this procedure with my insurance company due to not adhering to standard medical charting requirements.
