- Visiting Nurse & Hospice Home is a community-based, non-profit provider of hospice and palliative care, serving patients and families in Allen County and the surrounding area including Adams, Wells, Huntington, Wabash, Whitley, Noble and DeKalb counties.
Their hospice program provides compassionate care for the terminally-ill and support and assistance to the family via a team of professionals including: medical director, nurses, home health aides, social workers, non-denominational chaplains, volunteers and bereavement counselors.
Visiting Nurse & Hospice Home provides care in the patient's home, in a nursing home or in the agency's 14 bed in-patient unit, Hospice Home, the area's only facility dedicated to care for the terminally-ill.
Hospice care is generally paid for by Medicare, Medicaid and private insurance. Patients without resources or insurance may qualify for the agency's charity/sliding fee program. The majority of the patients cared for by Visiting Nurse & Hospice Home are elderly but the agency serves patients of all ages.
