    On April 24, 2024 I emailed Dr. ***** a *** approved neuropsychologist with Psych Health LLC at ************************************************************ about a test I needed to complete in order to be cleared to return to work. I received email correspondence from Dr. **** with an appointment date and the amount the test would cost $800 the same day.On May 9, 2024 I attended the appointment with Dr. ***** **** for testing after completing and emailing all his required patient forms. I completed testing with Dr. **** that date required by my employer, in order to provide a satisfactory test to be cleared to return to work. I paid a check in the amount of $800, the agreed upon price. Dr. **** said he would email me the records from the testing/appointment. Following this appointment, I attempted to reach Dr. **** many times (through May and June) for these records and sent a ROI June 10th via email and mail. I received no response. The attempts to reach him continued numerous times by myself, my primary care provider at the ******************* Health Care System, and an attorney through June and July. Dr. **** has never replied to any of my or their attempts to seek my records from this appointment to this date. He has never responded to me or emailed me back following May 9.I am seeking return of my repayment of the $800 for the testing, as I never was provided with records that were necessary to complete my return to work. Because of his nonresponse, I was forced to seek this test with another *** approved neuropsychologist, which cost me more money (for the same testing Dr. **** provided) and delayed my return to work clearance by three months. I passed the test and have since submitted all paperwork to return to work; therefore, I no longer need his records. I am simply seeking a refund, as he did not uphold his side of this transaction and unethically, has not responded to any of my attempts to reach him or receive my records despite completing a ROI.

    On June19,2024, I met with *********************** at his office at **************************************************** for an evaluation that I needed to complete to apply for an Airman Medical Exam for my pilots licence for my job at ****** I was referred to ************ by ***************************, ofElkhart, IN, an FAA HIMS examiner. I paid ************ ******* for the exam.I have contacted him repeatedly for a receipt and for the report. As of yet,I have received neither. I need these documents.
