      November 12, 2024 I sent a certified letter (notice of cancellation) to Elite Physical Medicine. I contacted them several times, even in person. They have not responded to my request as of yet.

      This patient originally called this office and spoke with Jeremy K**** on the phone. The patient completed all services a few years ago and has run into some personal difficulties with his loan payment now, a few years after completing all treatment. We directed him to reach out to his loan company, Proceed, with regard to questions about his loan. We had not received anything from Proceed or the patient since this call. We then received this complaint and another call from ** ********, where we got on a 3-way call with Proceed to help the patient to clarify his question. Proceed asked for our office to complete a form, which we have. This will get processed by Proceed and then it is back to ** ******** and Proceed to resolve the loan payment portion. From our companies view, the patient completed all services years ago and has run into hardship on the loan. This would need to be resolved with the loan company. There was no observed lack of response or communication between the patient and our office. We timely responded to all communication from the patient.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this does not resolve my complaint.  For your reference, reasons for rejection are included below.


      In the contract, it said that the balance will be re-amortized, and that did not happen, and I was not provided confirmation of this cancellation. Furthermore, the non-refundable deposit receipt has a date of February 25, 2025, on this letter and this date has not come yet, I am confused.

      The date signed on the non-refundable deposit was an error. As the document was signed in 2022 not 2025. However, it is irregardless. All services signed up for were delivered satisfactorily in 2022. The non-refundable deposit only applies to paying and then not starting and forfeiting that non-refundable amount. All services were delivered in 2022. So I don't see a refund owed as it has been 2+ years since all services were completed. Any issues with paying the loan were directed to the lender. Our office has satisfactorily completed services from a few years ago. If you have any other questions, let me know.
