    I requested my medical records report from ExamOne due to my disability insurance application being rejected. They provided me with the report on July 28, 2023, and the order number was ********. However, I noticed that there is a significant amount of information related to psychological disorder diagnoses that are not relevant to me. I sent a letter to the company to dispute this issue, but I have not received any response from them.

    Hello,

    ****** **** requested a copy of their ExamOne report on Wednesday, August 16, 2023. A copy of the ************* report was mailed on the same day Wednesday, August 14, 2023. We received an email on Tuesday, August 29, 2023 asking to open a dispute on some records on their report. A dispute was opened and sent to the data provider(s) for research. On Monday, September 11, 2023 we received an email from the data source to honor the dispute and remove the disputed records. On Wednesday, September 13, 2023 a copy of the new report and cover letter were secure emailed to the email address we received the dispute from plus hard copies were mailed to the address on file.

    There is PHI on the reports, so I am not able to attach to this reply. If ****** would like to contact me directly, I would be willing to discuss their new report.
