    Re: Hazel's Happy Home Daycare. Provider name: ***** ********** Phone ************ and business address **** ** ******* ***** ******* ** *****. State of Kansas Home Daycare License number ******. Her DCF Provider ID *******. Date of specific issue is Wed, June 14, 2023. The amt of today's specific issue is $220.00 was paid to this provider/business on Monday, June 12, 2023 confirmation no. ******. This home childcare provider had a signed contract agreeing to provider childcare services for my child, Monday through Friday from 7:30 am to 5:30 pm and would provide 2 weeks written notice if/when no longer able/willing to provider childcare services. This daycare has been closed or opened late or closed early at least once each week, and no refund or reduced rate honored for services not provided but contracted to provide. As of Monday, June 12, 2023 upon my payment to provider of $220.00 she refused to provide childcare services to me for the week paid for. Daycare closed last min for 6/13/23. On Wed, June 14, 2023 provider states she won't provide childcare services and told me to "take my baby and she was no longer providing childcare for us." She refused to return my child's personal items that I had to take to her daycare for my child's use. These items include: 4 baby bottles with nipples and lids, a full package of Pampers(48 count) size 3 diapers, a large package of ******* baby wipes, 4 complete spare changes of clothes(2 short sleeved onesies, 1 long sleeve onesie, 3 pairs of pants, 1 sleeper and 2 pairs of socks, 2 receiving blankets, a pack and play crib sheet, 2 pacifiers, small security blankie/toy, 6-4 oz/e. containers of Gerber baby food(1 squash, 1 pumpkin, 1 carrot, 1 pea, 1 green bean and 1 banana), 1 full container of infant rice cereal, 1 full can Similac sensitive infant formula, diaper rash cream, Baby Magic baby lotion, 2 infant feeding spoons, 2 small bowls with lids, child health assessment and immunization records, 2 toys.
