      My mother was an employee with a 401k with the company through fidelity. She passed away in August of 2023 and signature will not let fidelity give me any information regarding the 401k even though I’m the heir, and beneficiary. I’ve called them left voicemails and even tried to send emails to the CEO and none of the phone numbers have given resolution. Being the heir to her estate, I need to know where to send letters of testemtary so I can collect the 401k to pay bills for the estate and no one is available. This company has avoided me for months now in regards to a 401k that my mother put money in and is entitled to me by the court. I even tried emailing the ceo and the email address is invalid.

      ******* does not owe this bill. She was released from facility, I ask to pick her up. They said NO they would bring her home. I received a call they said they had a problem with transportation. I asked to pick her up I only live two blocks from facility. They said NO we will bring her home the next day. She was billed for two days that was not necessary. I could pick her up. I've never seen a place that would not let you pick up your relative. This bill is own them not her. I would like a zero balance and a letter stating a zero balance.

      I was in Tanbark Rehab facility, part of Signature Health Care, in Lexington from May 13th (4:00p) and left on June 2 (11:00a), but only part of each of these two days. I was discharged after a total of 20 days, a stay completely covered by my insurance, Medicare, and planned that way. Tanbark/Signature writes me that I owe them for one day, 194.50. June 1 is the date claimed, but the 20 covered days extended through June 1, so I do not owe them anything. Kelly White is the business manager at Tanbark. I explained all this to her in my detailed letter of August 1st and a few days later she telephoned me. She said to disregard the bill, that I don't owe them anything. I thought the matter was closed. However, I'm still getting a bill for one day, without explanation. It is likely that Tanbark/Signature Health is "surprise" billing me to get some extra money. The Tanbark address in Lexington is **** ******* ***** ********* ******
