      To Whom It May Concern,
      Complaint Regarding Refusal of accommodation
On 5/21/25, I attempted to submit paperwork necessary to dispute a charge of $1208 (x2) on my account, as is my right under the Fair Credit Reporting Act (FCRA). Due to my disability, I requested the following reasonable accommodation: , "to submit the documentation electronically via a secure portal," This request was denied by Shawn, who stated that they do not use email to correspond, even if it is requested as an accommodation.
This refusal constitutes a violation of the Americans with Disabilities Act of 1990 (ADA). Title III of the ADA prohibits discrimination on the basis of disability in places of public accommodation, which includes financial institutions like Credit Clearing House. Covered entities are required to make reasonable modifications to policies, practices, or procedures when necessary to afford goods, services, facilities, privileges, advantages, or accommodations to individuals with disabilities, unless doing so would fundamentally alter the nature of the goods or services or result in an undue burden.[1] The requested accommodation was reasonable and necessary to ensure my equal access to your dispute resolution process.
Furthermore, this refusal has directly impeded my ability to exercise my rights under the Fair Credit Reporting Act (FCRA). The FCRA grants consumers the right to dispute inaccurate or incomplete information on their credit reports, and furnishers of information, such as credit clearing house, have duties to investigate such disputes.[2] By denying a reasonable accommodation for the submission of required dispute documentation, Credit clearing House has effectively prevented me from initiating or completing a legally mandated dispute process, thereby violating my rights under the FCRA. The Consumer Financial Protection Bureau (CFPB) enforces federal consumer financial laws, including aspects of the FCRA, and emphasizes fair treatment

I do not know who the collector or the original creditor is. I would like to request a debt validation of debt to verify that this is my debt. I have never received one. I never signed with a collection agency. I have no recollection of the creditor reaching out to me at all. I would like these derogatory remarks off my credit report. If they are unwilling to do that, I need a debt validation of debt where they can verify every bit legally required information that the debt belongs to myself and not someone with the same name or someone opening the account fraudulently. Thank You.

Collection agency trying to have me pay a bill i already paid long time ago have proof, this shouldn't be on my credit report. I shouldn't be billed twice for same bill? When I go to website for emergency medical associates shows I owe nothing! I have bank statements showing this bill was paid long time ago.

This is in response to your claim that a payment of $225.00 that you made in May 2023 paid this claim .
We have attached a copy of information from the original creditor. One bill for $225.00 for service dating from
I I /23/2022 and a second bill also for $225 .00 for services dating from 4117/2023. As you can see the 4/ 17/2023 bill
credit was given for your May 2023 payment. If you have proof of payment for the II /23/2022 bill please send that
to us and we will investigate the matter further.
Please compare this information to your payment records. If after you review the creditors' information you find you
disagree with the amount owed, please send us your side of the story, include any documentation that supports your
side of the story. If you respond to this letter, we will investigate the matter further.

I received a bill dated April 21, 2023 from Credit Clearinghouse of America, Inc. ( a company I have never heard of). It listed a bill for $2240.00 from "Pinnacle Inpatient Medical Services", another company I do not know, from September 24, 2020. There was no other explanation of this money they say is owed. There is NO email address anywhere on their website. The LinkedIn for the CEO did not list any contact information except reference to the company website. FROM THE LETTER : Reference:*******, Account Number: *****.

We need more information on this individual to be able to identify the correct account. We do not have an account with that name and address or phone number to match it to. We need the last 4 of a ss#, an account number and who the account is with. Without more info there is no way we can respond to this "complaint".
