      ORDER EYE GLASSES WENT TO PICK UP THE EYE GLASSES THE EMPLOYEE INFORMED ME THE EYE GLASSES WAS CRACKED SPLIT FROM DURING LENS PLACEMENT THE LENS HAD A SCRATCH, THE MANAGER TRIED TO MAKE ME TAKE THE EYE GLASSSES, I SAID NO ! I REQUEST REFUND BACK TO PASSPORT MEDICAID BY ****** ********* SO THAT I MAY CHOOSE EYEGLASSES AT ANOTHER STORE NEAR MY HOME. THEY DID NOT REFUND MY INSURANCE WHEN I WENT TO VISION WORKDS THEY SAID THE BENEFIT WAS PAID TO GADDIE EYE CENTER SPRINGHURST LOUISVILLE KY 40214. I WENT IN FOR MY YEARLY EYE EXAM THEY TRIED OVER CHARGE ME $45.00 I DID NOT AUTHORIZE ANY CHARGES OTHER THAN YEARLY EYE EXAM. DIABETES EYE EXAM. THE MANAGER HAS NOT REFUND MY INSURANCE AND I WAS NOT ABLE TO BUY ANOTHER EYEGLASSES AT ANOTHER STORE. SHE STILL REFUSED TODAY AFTER I GOT VISION EXAM I TOLD HER NO I WILL NOT ORDER ANYTHING FROM GADDIE EYE CENTER SPRINGHURST LOUISVILLE KY 40214. GADDIE EYE CENTER NEEDS TO REFUND MY MEDICAID INSURANCE TODAY! THEY ARE NOT KEEPING MONEY THAT I DID NOT GET ANY EYE GLASSES.

      Unsatisfactory resolution of properly fitting glasses. First new pair of lenses we made wrong. Glasses reordered. Second new glasses the lens on right eye was fitted wrong by manufacturer. Third pair picked out with the help of staff and ordered but the bows were to long but they would not help any further because out of warranty. I asked that my complaint be elevated to the manager who was to call me. Never heard from anyone. Now I'm the caregiver for my who needed a lot of care over the past two months so I've let this slide a couple months and prove it with paperwork. I'm at a lose as to why after all these errors by Gaddie and their supplier that providing shorter bows was refused. My original eye exam was in July 2023.

They have replaced my glasses with a new pair that i am thrilled with. All at no charge.       On June 23, 2022, I took my daughter to a Gaddie Eye Center for an eye examination. I was charged $50.00. I paid the $50.00 using my ********** and received a receipt for this payment. See attached receipt from Gaddie Eye ********.

      Since June, I have received numerous statements from Gaddie Eye Centers informing me that I still owe $50.00. Twice, I have called the number at the top of the statements (************) to inform the company that I paid the $50.00 on June 23 and have both a Gaddie receipt and a ********** statement showing the payment went through. I continued to receive bills so I drove to the Gaddie Eye Center in ******** to show my proof of payment. I was told this information would be provided to the manager.

      I continued to receive bills for $50. On October 14, I called Gaddie Eye Center (************) and spoke to ******** She said that only ********* ********* could help me and I would need to call back on October 17.

      I spoke to ********* on October 17. She requested that I email her at *****************.com with a copy of my ********** statement showing that I paid $50 to Gaddie on June 23. I tried to email the statement to her multiple times, but the Gaddie server rejected each email.

      I gave up on emailing ********* and drafted a letter instead. On October 21, I hand delivered the attached letter to Gaddie ********. I still continue to receive bills for $50 from Gaddie so it is obvious that the $50 charge has not been removed from my account. I have attached a copy of the most recent bill (dated December 6, 2022.)
