    I called Chism Funeral Home today requesting a statement of my dad's funeral services dated within the last 90 days per ******************. (reference BBB complaint ******** against ******** **********.) They state in order for me to reimburse myself for my dad's funeral and cremation services out of my own 401k, I must provide a STATEMENT, NOT altered receipt, within the last 90 days. They state this is an IRS requirement. I spent 30 minutes on the phone with a ******** Customer Service Supervisor discussing this.
    When I contacted Chism Funeral Home today they refused to provide me a statement within the last 90 days. I clarified I am not asking for an altered receipt and explained what ******** ********** explained to me.
    I've had two deaths in my family this year and just want to be able to reimburse myself through my 401k. My dad died December 2021, services were rendered by Chism in January 2022. They collected $2100 and there is no reason why I can't be provided a STATEMENT per ******** ********** to reimburse myself.

    ********* was given a paid funeral bill at the time the bill was paid so technically we are not required to do anything beyond that but she did get a statement that day with a current date so she could give to ******** because we always try to do right by our families.  She called and talked to one of our employees who I think was confused about what she was wanting and he doesn’t really deal with the billing and she was not willing to wait until a call back from one of the owners before she made the complaint.  The owners were out on a funeral and the employee tried to take a message for one of us to call her back.  Once I got into the office, seen an email from her stating she had filed a complaint, wanted a statement and  I talked with our employee I called her.  I clarified what she wanted and emailed it to her. I also tried calling her several times to make sure she received it but got no response back from her but I assume she did receive it or I would have heard from her again. We have never had a complaint on us and take complaints very seriously. She has filed two with two different companies..I know in our case she received what she needed once she talked to management so maybe  she not be so quick to file a complaint.  Thank you.
