      I went in for voluntary anger management classes. The teachers were unqualified interns with no degrees or accreditation in counseling (the teacher was pursuing his degree). There were mandatory urine tests even though I was there voluntarily. I didn't mind doing it but they lied to me about the cost. Basically each test was around $200 billed to my insurance. In an 10 week course that's $2k. In the beginning they told me I wouldn't have to pay for the tests and had me fill out a hardship form. They told me to lie about my income and then I wouldn't be billed. After about 6 weeks into the class I got a bill from the medical diagnostics company for the first test. I owed about $200. I brought this up with their staff and they had me fill out another hardship form. Again they told me to lie about my income. When the class over I was on the hook for $1600 worth of unnecessary urine test bills. I tried working with the manager to get my medical bills reduced or have them help but she said there was nothing she could do. In the end she basically told me to go back and read all the waivers and fine print that I signed upfront and that I was legally liable since I signed it. I ended up settling with the diagnostics company for around $800.

      **** **** enrolled in services with the knowledge and understanding that drug testing was a required part of our program. He was well informed that he would be responsible for any and all costs associated with the program and drug testing. Prior to starting services **** was provided with and signed the document "drug testing policy" which explains these responsibilities. This information was available for his review anytime in his client portal and also provided in the client handbook he received. The policy is also visibly posted in the office.
      For clients that are uninsured or have insurance deductibles that have not yet been met and would likely incur charges for the cost of drug testing, we do have a hardship waiver that is provided to us by the lab we use "ACCU Reference Labs". The hardship waiver is a request to waive fees, but ACCU Reference Labs is ultimately the decider of who is eligible. ACCU Reference Labs is a separate company not affiliated with Active Recovery that provides laboratory services. ACCU Reference Labs has its own policies, procedures and billing practices separate from Active Recovery. It was explained to **** that he would have to work out the billing issues with ACCU Reference Labs because since ACCU Reference Labs is a seperate company, Active Recovery cannot dictate the billing practices of ACCU Reference Labs.
      Active Recovery denies the allegation and has no knowledge that any staff members encouraged **** to "lie" on the hardship waiver and does not condone the falsifying of any records.
      The program in which **** was enrolled in, is a psycho-education group that does not require any specific degree or accreditation to facilitate. Active Recovery provided program specific training to the facilitator of ****'s program in addition to the bachelor's degree that the facilitator holds. The facilitator is enrolled in a masters program and has several years of related professional experience.

      While I did sign the drug testing policy I was absolutely told to put an income of zero on the hardship form multiple times. I was led to believe this would cover my expenses. This seemed like SOP but I have no way of knowing how they handle other people. They may deny the allegation but I know for a fact that I was directed on several occasions to fill out the form with false information. As I have no way to prove this I suppose it is my word against theirs.
