    As part of a Collaborative Divorce settlement, the *** firm came recommended to us by the Financial Advisor on our Divorce settlement team (******* ******). Mr. ******* ********* handled our joint tax return for 2022 by filing an extension and filling out relevant items for 2 states. The expectation was for us to receive confirmation that federal and state tax forms would be confirmed as filed by 10/15/23. On 10/15/24, I was notified by the *** and **** DOR that these documents were NOT filed as expected. In November 2024, I received a CP14 notice detailing penalties and interest. I had notified the *** firm of this oversight in October but no pro-active action to prevent CP14 notice generation was taken

    We have thoroughly reviewed Mr. ************ file and the notices submitted. It has been determined that the electronic filing system did not receive the tax return; however, we acted quickly by coordinating directly with the *** to address the notice. As a result, all associated penalties and interest were abated.


    Moreover, we would like to clarify that the filing issue has no bearing on any divorce proceedings or personal matters Mr. ********** may have. Our preparation of the tax return was completed accurately, and any issues concerning electronic transmission were resolved promptly and in full.


    Given these findings, we consider this matter closed. Mr. ************ request for a refund will not be honored, as all services were performed competently and effectively, and we find the claims made are inflated.
