This regards ********************* who is deceased. An unsigned letter dated July 10, 2023 regarding a refund check (# ****** for $1,188.45) was received by the co-executor of his estate. The requested document was returned to appropriate address. No further communication was received and repeated telephone calls and an email enquiry have not resulted in additional information. We are attempting to close the estate and need to resolve this matter. The letter states that, should the response not be received the funds would be reported as unclaimed property. A search of the state unclaimed property website reveals that no such file is included in the listings.

I received a bill from ******** dated 06/03/22. At the time of receipt, I called the company to ask why there was any outstanding balance, as every copay was paid. I left a message with the account number, name and phone number for them to return the call. The company never responded. Since that time, they have sent repeated bills for the same amount with no explanation as to reason for bill. I have left over 10 messages at the phone number specifically listed on the bill *************. I have a log of dates and times each and every call was made over the course of the last 2 and a half months. They REFUSE to return any calls or answer any phone lines. It has been impossible to get any person to speak with. They expect you to just pay the bill and not question it. I refuse to pay a bill that I don't believe I owe and that they refuse to discuss with me. I spoke with my medical insurance company and they have told me that they do not show that I owe any additional copay. I have a case number from them where they have documented our discussion and the fact that they show that i do not owe this bill.
