    Over the course of June and July 2022 I received a root canal and crown treatment from Arch Dental and have, over the course of the following year, needed to have every aspect of this procedure retreated on the same tooth. The re-treatment of the root canal at nearby specialist ********************* Endodontics was over two thousand dollars and not covered by insurance because of redundancy, and the new crown just installed by my new dentist was nearly two thousand after insurance partially covering the treatment. Ideally my refund from Arch Dental would include or consider these additional costs in repairing their faulty medical treatment.

    We have not received any correspondence from the treating office to indicate reason and actual retreatment and remake of crown. At this point we are not able to make a decision without the information. Please provide clinical notes, billing information and x-rays. please use my email for correspondence ********************

    I am rejecting this response because: this issue is not yet resolved. I will work to provide the requesting billing and medical information from other practices I have since sought treatment at but would also point out that a doctor at Arch Dental initially assessed the progress of the procedure and recommended I have the root canal retreated by a specialist because it was incomplete/ineffective. I trust this information would be in your office's medical records of me. Thank you for your time and consideration.

    It is necessary to review the clinical notes from the treating specialist to determine their findings and clinical necessity. It is a reasonable request that would be required for us to make an informed decision.
