    I have been a patient with Option Care Health since October 23, 2024. During my intake, I asked if they offered financial assistance. The man I spoke with, said yes, and that he would have them send the application paperwork. When my patient care coordinator called me the following week, to set up my next delivery, I asked again. She said she would reach out to financial assistance, and have them reach out to me. Over the course of the past few months, I have called billing multiple times, requesting the *** paperwork. My patient care coordinator, and even the pharmacist has reached out on multiple occasions. Finally, on 2/3/25, someone called me, and said that they would email me the forms. On 2/6, I still hadnt received them, & I called that number back, and the line was disconnected. Meanwhile, the bill is growing. I make payments every few weeks. My most recent payment was on 4/8/25. On 3/20/25, I called the billing department again, explaining that I still hadnt received the forms. They claimed that I was emailed on 3/14. I confirmed my email and address. I still havent received the forms! I dont understand why these forms cant be mailed to me! Paperwork is sent with my weekly IV medication delivery all of the time. Why arent these forms printed off and easily available to the pharmacy sending me my meds every week?!?!? All I would like is to receive these forms. I really need the financial assistance. It shouldnt take 6 months to receive financial assistance application forms! Please help!
