    After several attempts to contact the provider for a simple referral which is internally within WVU Medicine, I have yet to receive a response from the office. It is definitely a lack of continuum of care. It has been since December 20th of my last dosage nonrenewal of prescription. It took some time prior to now to find the place which will provide continuum of care of weight loss medications. I am just in awe of the lack of responsibility and accountability of my continued care from VA to MD. it has been less than subpar. The Department (******) of which I need (referral to be seen) to attend for the continuum of care has called, the insurance company called **** ****** on the 14th and I have called three times along with sending a fax. This perplexes me as to why it is such a challenge and or complicated to write what is template for my continued care. Correct if I'm wrong but I thought a doctor took an oath to "do no harm." From my point of view, I am being ignored (though I yell to the top of my lungs) I am mentally exhausted and physically lacking my medical treatment for me to qualify for breast reduction while I am continuing my weight loss journey. All I need is for the office to fulfill a request for me to see the office to offer me continued treatment of which I am well over 30 days in lack thereof.

However, in the event that she is an active patient at any of our locations, her complaint will be referred to the *************************** for investigation and appropriate follow-up. In general, we make every effort to promptly address patient requests for referrals. We provide the name and contact information of the referral specialist, and it is the patients responsibility to schedule the appointment at their convenience.

    My credit score dropped 45 points on Equifax staying I have a 64 dollar past due bill with this facility. I haven't even been to this facility.

    Garrett Regional Medical Center (GRMC) received notification of this complaint on January 20, 2023. This was the first time we were contacted about this compliant.

    The timeline of events from our ************************************* is:
    > Specimens for ordered blood tests were collected on 12/20/2021 at patients resident facility.
    > Specimens were brought to GRMC for processing for Aeon Technologies as a mobile lab service.
    > First statement was sent to patient on 1/24/2022 in the amount of $64.14.
    > Statements were sent to patient monthly on 2/23/2022, 3/25/2022, 4/25/2022, 5/25/2022, and 6/24/2022.
    > Account turned over to collection agency on 7/21/2022.

    GRMC has not been contacted by anyone in relation to this account.  GRMC inquired with the collection agency if they were contacted about this account and they advised that they have not been contacted either.  The collection agency states that they were notified that a dispute form was filed with the credit bureaus on 1/6/2023 claiming fraud but that was the first notification that they received about this.


    GRMCs process for these mobile lab service accounts is that the specimens are brought to our facility along with the orders for the tests.  The tests are processed and the bill is sent to the patients insurance company that is on file.  This patient did not have any insurance information listed.  The bill was sent to the patient for payment.  GRMC was never contacted with information about an insurance carrier for the patient to bill for the tests.


    Documents demonstrating that this process was followed are being submitted with this response. These documents are:

    > Statement detail with all activity on this account.
