    Now i am receiving 10 phone calls a day plus emails from debt collectorsted message telling you to call back. I would like this matter resolved and corrected.

    Hello, The National Spine and Pain Center employee has insert incorrect informtation into the patients medical record and the request for Amendmednt had been request in writing and givent to Mr. Chris H***** in July 2022. The National Spine and Pain Center has failed to correct the medical record of the incorrect inaccuate medical inforamtion contain in the medical record. On Septemeber 2, 2022 the form was given to Mr. H*****s. He recieved the REQUEST FOR AMENDMENT OF PROTECTED HEALTH INFORMATION and he recieved the supporting document in person in July 2022. Initial on July 18, 2022, the Practice Administrator, Mr. Chrisptoher H*****s, of the National Spine & Pain Centers was provided n writing the request to amend incorrect/inaccurate medical records. Please see attachments. Thank
