    Business Response

    Date: 05/15/2023

    Dear BBB and ******************,

On 5/15/2023, I received the first email correspondence from the BBB notifying me of the complaint raised by ***************** *******.  ******************** does record procurement for attorneys and insurance companies, we are hired by attorneys to collect records they need to support their cases.  We are a non-bias, third party who works for both plaintiff and defense attorneys across ********  Our process is to prepare authorizations (HIPAA, employment, etc.) to be signed by the plaintiff.  We issue those unsigned authorizations to the plaintiffs attorney who then has their client sign them giving LCS permission to obtain records.

Ms. ***************** ******* was involved in two lawsuits in ********, and my company was hired to obtain her records.  As is customary, we issued the authorizations to ******************** attorneys to have them signed. ****************** did in fact sign the authorizations, and we have proof that the signature was not forged.  The signature on the authorizations provided to LCS matches exactly with the signature on ******************** drivers license. Likewise, we have many forms signed by ****************** confirming that the signature is a match.

It is likely that ****************** is not aware of the role that LCS plays in the gathering of records.  ****************** probably signed the forms her attorney provided her and did not pay much attention to the fact that LCS was going to work on the gathering of records.  LCS has encouraged ****************** to discuss this with her attorneys, which it appears she has not done.

    Case and attorney information:
    *************************************** v ***************************************** 20-012409-NF 3rd ** Circuit Court
Represented by Attorney *********************************** of Michigan Injury Advocates, PC **************
    ********************************** v ****************************** and ********************************* 22-008644-** 3rd ** Circuit Court **************
Represented by Attorney *********************** of **** and Associates, PLLC

My company has previously discussed this with ****************** but I am available for further conversation if she would like to contact me.  I will be contacting ******************** attorneys to inform them that they should contact their client and explain the situation.  Please let me know if you need further information to resolve this issue.

    Sincerely,

    ***************************

I am rejecting this response because: I already spoke to my attorney and they never got this form from LCS nor they gave it to me for me to sign it. This is definitely my signature and is not close to how my signature is on my driver license. Believe me I do check and read each and every form that I signed and believe me I never ever had that form on my hands this is a forged signature and if LCS didnt forged my signature I need to know who did. My attorney said that they never received that form and obviously they didnt have it to me for me to sign. Someone here is lying and is not me . Believed me I know my signature and definitely is not my signature.

    Sincerely,

    ***************** *******

    Dear ******************,

I understand your concern, and I apologize for all this confusion and back and forth.  We have proof of sending these blank authorizations to your attorney multiple times between 9/15/2023 and when we received them back signed on 1/10/23.  I have the email where *************************** returned the signed authorizations back to LCS.  I can send you copies of all those emails but they do include Personal Identifiable Information (PII) so I must do so securely via encrypted email.  I have your email address as ********************, is that a good email address to use?

Again, I ensure you that LCS did not and would never forge a signature.  I hope that through the data that I provide, you will be able to see that LCS has done nothing of the sort.

    Sincerely,

    ***************************

I am rejecting this response because: I need to see those emails first , before accepting your response. Could you please send me the proof that you received this signed document from my attorney, and all the forms you said *** signed for LCS before. The email you got for me is the correct one you can send it to that one. Thank you

    Sincerely,

    ***************** *******

    Business Response

    Date: 05/25/2023

    Dear BBB and ******************,

I have responded to ****************** via secure email transfer outside of this platform.  I have email ****************** proof that we received the signed authorizations from her attorney.  I ask that with this proof provided, ****************** kindly close this BBB complaint as resolved.

Emailed sent securely on 5.25.23 at 8:50AM which included 4 attachments:

    Hello ******************,

Per our conversation, here are the email showing proof of receipt of the signed authorizations from *************

The three emails with subjects starting with "Response_documents required" are just a few of many emails to **************** office seeking his client's signature.  The fourth email with the subject *********** ******" is where ************ attached the signed authorizations and returned them to LCS.

As you can see, this is proof that LCS did not forge your signature.  I would appreciate your kindly dropping the BBB complaint with my company.  If you have any further questions, I would be happy to help answer them.

    Sincerely,

    ***************************
