    This business assisted with emergency IT issues - he will not submit his W9 or provide his EIN for year end tax purposes. We have called, emailed, and stopped at his office - he will not provide us any information nor will he call back.

    She is correct.  This is our fault and was the perfect storm of circumstances coming together to create this problem.  She only had contact info for our service engineer.  He was not responding.  We moved our office 6 months prior to a new location.  Not knowing that, she drove to our previous office.  Ironically, this service engineer was on site meeting with the company in our previous office when she stopped by to find us.  He did not pause the meeting to see her.  She did not have the contact info for the new office, nor did she know about it.  Our office found out about it first by ****** review she posted.  We contacted her within 24 hours of the review she posted.  We apologized.  Explained the situation.  Gave her the office contact information and sent her the statement she needed.  She seemed very appreciative.

    This was certainly our fault.  Very sorry.  We will work on systems to prevent this from happening in the future.
