      Dr. ******** *****, DO has VERY POOR patient check in procedures: At my appointment in January 2025, I was asked to read through a several page digital-electronic version of Dr. ******* "Physicians Patient Agreement" and digitally sign it in agreement. However, when I asked for a copy of the agreement for my file, the staff refused to provide a copy of the agreement. I asked to speak to Dr. ***** and was refused, I left 4-messages requesting Dr. ***** to call me regarding this matter. To date Dr. ***** has not called me. I have a Legal right to a copy of any document I am asked to sign.

      I contacted the costumer/patient on 3/5/2025, acknowledged his complaint and apologized for the misunderstanding and inconvenience. I assured him that our policy is to provide all documentation applicable to him upon request and that I would be ensuring a thorough internal investigation into our procedures and reeducation of our staff to ensure this does not occur again. The costumer/patient indicated that I was welcome to contact him again once the reeducation was complete and he would consider returning to the practice, in the meantime he was able to seek care elsewhere. I thanked him for bringing this to our attention and for the feedback so that we can continue to improve.

      I have reviewed the response made by the business in reference to complaint ID ********, and have determined that this proposed action would not resolve my complaint.  For your reference, details of why I am rejecting this response appear below:

      No specific action was indicated on how I would receive copies of signed documentation, or when this issue would be resolved, no compensation was offered for my 2+ hours of wasted (and still not being able to see Dr. ***** at the appointed time), and why Dr. ***** did not return my calls (after leaving 4-messages).
