  My Grandmother lived at the Diamond Willow Assisted Living facility branch in *************, ** from 2020-2023. She was physically, mentally and emotionally abused and neglected. Staff at all levels were responsible for her maltreatment. Not only were the personnel not medically certified at the correct level of care, including the director of that branch, but the administration was notified multiple times over the course of 3 years of the abuse and neglect inflicted upon my 76 year old Grandmother and took no action to ensure her health and safety. After she moved to a separate facility, she was found in extremely poor condition and her reports of being 'punished' by being forced to sleep in a wheelchair, spend multiple days in bed without moving and receiving little, moldy or inappropriate food have been substantiated. Attempts by family members to check in with my Grandmother were met with hostility and overt interference with visits and phone calls. At one time, I attempted to call my Grandmother for 3 months both directly and to the staff desk without receiving an answer. It was also discovered that many of my Grandmother's personal items 'went missing', disappeared or were flat out stolen. Many of her narcotic medications were unaccounted for and she received physical injuries from incompetent staff such as hot coffee ***** and falls onto her head that were unreported and for which she did not receive recorded medical attention. During visits, I observed that many of the other residents in the facility were being treated and suffering the same. The ************* branch of this facility should be avoided until/unless major staff and facility changes are made.
