      I had tooth extracted on December 4 2024, simple procedure no anesthesia. They demanded payment upfront the highest on their scale. They also filed my insurance and was paid in December along with my payment of $350. I am due a small refund per my insurance and office but the have yet to send it to me. I have called numerous times after trying to bill me for the office visit that they didnt file until April 2025. Only after I called ***. We have called Springfield and the ******* offices trying to get this matter settled. They demanded I pay on initial visit (overpaid) but they are reluctant to send me refund due me after insurance paid along with my payment.

      11/14/2024 date of service was filed to insurance on 11-19-24.

      Prior to services being performed on 12/04/2024, the patient signed the treatment plan including charges for both dates of service. It does note that this is an estimate only. The patient consented to proceed with the surgery. This date of service was also filed to insurance on 12/05/2025.

      We received payment from insurance for 12/4/2025 on 12/13/2025. We still had not received payment from insurance for date of service 11/14/2025.

      A statement of the account was sent to the patient on that day. Another statement was sent on 1/20/2025. A call was made from our office to the patient on 3/18/2025, no response from the patient. A phone call with the patients wife on 4/03/2025, resulted in receiving updated insurance information. Prior to this, we could not access the patients account with the insurance company, as they had made a change in their computer system after the first of the year.

      After researching, no claim was found with insurance for 11-14-2024 date of service. A new claim was filed. Payment was received from insurance on 4/16/2025, resulting in the first overpayment on the account. A refund check was made out to the patient and mailed to the home address we have on file, on 5-21-2025 in the amount of $43.02, bringing the account to a $0 balance.

      On September 3, 2024, my husband was referred to get his wisdom teeth surgically removed and this was the day they gave us a treatment plan. We were also told that because our insurance is based in ********, they could not file or help me fill out my reimbursement form. I called in October 2024 to ask for the TIN and the billing lady I spoke with was very rude saying we signed a paper that we wouldn't go and file for a reimbursement ourselves.Little did they know, I have several years experience from my profession in filing insurance, knowing where to find the ***** NPI numbers, and TIN. I then told her on the phone that I knew how to do so, and it's my right so I will. Ironically right after that call, they billed my insurance in December after saying they had no way to file. When I called my insurance company, they told me that my insurance was billed by the dental clinic and paid on January 2. I contacted *** billing and they were insulting again saying that its impossible for them to file. Again, I'm not stupid, the clinic I work for in ******** has many patients with out of-state insurance, and we can still file them. The conversation ended with her raising her voice at me saying I was assuming things about the clinic that was true and she would refuse ant reimbursement to me

      I have braces to straighten my teeth. I was told that if I don't get my wisdom teeth removed, then when they grow in and it would move all my teeth back to the original location before I got the braces on. So, I went to see OMS in ******** to get an estimate on how much it would cost to have my wisdom teeth removed. I never got that estimate and they want to charge me for it. Since then I have moved back to ********** to live with my parents.My mom called OMS on 8/25/22 @ 2:22 PM PST (5:22PM EST)Left a message OMS Called on 8/25/22 @ 2:31 PM PST (5:31 PM EST)My mom called OMS on 8/25/22 @ 2:50 PM PST (5:50 PM EST) No message left My mom called OMS on 09/01/22 @ 8:34 am PST (11:34 am EST) Left message OMS Called on 09/01/22 @ 8:50 PM PST (11:50 PM EST)************************* specifically asked if there was a charge for the consultation and was told "NO" at the time she went in. ************************* didn't sign any documents saying that there would be a charge for the consultation ************************* didn't have x-rays taken for the estimate to see how much it would cost to have the wisdom teeth removed ************************* didn't get any price on how much it would cost - the whole reason she went in there in the first ********* still wants a $101 payment due immediately, payment on account or bill will go to collections
